Job Title: Hospital Case Worker

Duration : 13 Weeks contract

Work Location:  NYC, NY

Job Summary: 

Navitas Healthcare, LLC is seeking Hospital Case Worker for an exciting job in NYC, NY 10016. Shift - 9:00 AM-5:00 PM.

• Provide direct nursing care to residents in accordance with their individualized care plans.
• Assess client needs through interviews, evaluations, and case reviews.
• Develop individualized service plans tailored to client goals and needs.
• Provide counseling, guidance, and referrals to social services, healthcare providers, and community resources.
• Monitor client progress, adjusting plans and services as necessary.
• Advocate on behalf of clients to ensure access to essential services and support.
• Maintain accurate and timely case records, reports, and documentation in compliance with organizational and regulatory standards.
• Collaborate with multidisciplinary teams, community organizations, and government agencies.
• Handle crisis intervention and provide immediate support when necessary.

Qualifications:

• Bachelor’s degree from an accredited college or university (required).
• Minimum of 3 years’ experience as a Hospital Case Worker or in a related social services role.
• Strong knowledge of community resources, social services, and case management practices.
• Excellent communication, counseling, and interpersonal skills.
• Ability to manage multiple cases with strong organizational and time-management skills.
• Commitment to maintaining confidentiality, ethical practice, and client-centered care.
